1. Client Responsibilities

: a. Provide accurate and complete medical history information. b. Follow medical advice and treatment plans prescribed by our healthcare professionals. c. Notify the clinic of any changes in contact information promptly.

2. Appointments:

a. Appointments can be scheduled online, by phone, or in person. b. Cancellations must be made at least 24 hours in advance to avoid cancellation fees. c. Late arrivals may result in rescheduling of appointments at the discretion of the healthcare provider.

3. Confidentiality:

a. Client medical records are confidential and will not be disclosed without the client’s consent, except as required by law. b. Electronic communication, including email, is not guaranteed to be secure. Clients should avoid sending sensitive information through unsecured channels.

4. Payment and Insurance:

a. Payment is due at the time of service unless other arrangements have been made. b. The clinic accepts various insurance plans; clients are responsible for understanding their coverage and co-payments. c. Uninsured clients may be eligible for discounted rates based on financial need.

a. The clinic may communicate with clients via phone, email, or text message for appointment reminders, test results, and other relevant information. b. Clients can opt-out of non-essential communication by Replying STOP but are encouraged to receive important health-related updates.

a. In case of a medical emergency, call 911 or go to the nearest emergency room. b. The clinic should be notified of any emergency room visits or hospitalizations as soon as possible.

7. Termination of Services:

a. The clinic reserves the right to terminate services if a client engages in disruptive behavior, fails to comply with treatment plans, or violates clinic policies.

9. Governing Law

: a. These Terms and Conditions are governed by the laws of Oklahoma. Any disputes shall be resolved in the courts of Oklahoma.
